Functional Leg Length Discrepancy Pattern Secondary to a Left Torsional Sphenoidbasilar Strain Pattern

Why heel lift therapy should not be used

Due to the twist in the meningeal dura as it descends from the occipital magnum to the sacrum, a left torsional strain pattern at the SBS will side bend the pelvis to the right.  This pronates the right foot.  The body compensates by supinating the left foot (allowing the left foot to remain on the ground).  A side bent pelvis is seen on a standing X-ray.  The prone patient will demonstrate a short left leg.

Fitting a heel lift on the left shoe will (1) lock the pelvis in the side bent position, (2) lock the SBS in a left torsional strain pattern and (3) lock the right foot in a pronated position.  The supinated left foot may or may not reduce.  The patient may feel more level but the subjective symptoms will continue to exacerbate.

In a right Torsional Sphenoidbasilar Strain Pattern, the above pattern is reversed.
